    PB0099 - Thrombin generation parameters results in COVID patients comparing with healthy volunteers.

    Background: SARS-CoV-2 caused global pandemic of coronavirus disease 2019 (COVID-19), responsible for millions of infected with a high mortality index. Up to 30% COVID-19 patients in intensive care evolves with serious thrombotic complications. Due to this increased thrombotic risk, the administration of anticoagulants is mandatory, subsequently a bleeding adverse outcome can occur. In this scenario, global hemostatic assessment devices in laboratory department, such as thrombin generation, may predict complications and guide therapies.

    Aims: In this study, we analyze the main thrombin generation parameters in COVID-19 patients in the moment of the entrance in a critical care and three days after comparing with healthy volunteers’ parameters.

    Methods: Twenty-eight patients admitted to the intensive care unit for a SARS-CoV2 infection were included. Blood tests are collected, and thrombin generation tests were measured with STA-Genesia using TG-ThromboScreen reagent (Stago) in ICU admission (D0 – baseline) and three days after (D3). The control group are formed by 22 healthy volunteers. Statistical analysis were performed using multiple comparison test by Bonferroni.

    Results: On D0, the lag time were higher in COVID patients when compared with healthy volunteers. However, the ETP inhibition showed a worse behavior (table1). On D3, more parameters showed statistical difference to the healthy volunteers, with higher lag time, peak and velocity and impaired ETP inhibition. Correlating with heparin administration, patients that were being heparinized had LAG TIME more prolongated when compared to healthy patients and COVID patients without treatment. However COVID patients without anticoagulant treatment had higher peak and worse ETP inhibition. Finally, on D3 patients who survived had more prolongated lag time and better results for ETP inhibition (graph 1).

    Conclusion(s): Thrombotic and haemorragic complications are critical for COVID patients and identify the risk of this patients is complicated. The thrombin generation test, mainly LAG TIME and ETP inhibition, can be useful in monitoring critical patients.
