    PB0091 - Fibrin monomers positivity during hospitalization is a good predictive marker of in-hospital mortality during COVID-19

    Background: Coagulopathy is one of the main triggers of severity and outcomes in Coronavirus disease 2019 (COVID-19) in particular in critically ill patients. D-dimer has been widely used to detect COVID-19 coagulation disorders and outcomes. Involvement of other fibrin degradation products, and in particular fibrin monomers (FM) is still an ongoing question.

    Aims: Aim of the study was to assess the ability of FMs monitoring during the first 9 days of hospitalization to predict COVID-19 outcomes.

    Methods: During the first wave of COVID-19 outbreak, 246 patients hospitalized in medical ward (MW) or in intensive care unit (ICU) were included in a monocentric study. To assess the ability of FMs monitoring during the first 9 days of hospitalization to predict COVID-19 outcomes, we analyzed the positivity of FM levels.

    Results: In our cohort, 935 FMs measurement were performed in the 246 patients during the first 9 days of hospitalization. During patient’s follow-up, FMs levels were higher in patients admitted directly in ICU than in patients admitted directly in MWs. Moreover, we observed significant increase levels of FMs in patients when stratified on survival. At hospital admission, only 27 patients displayed a positive value of FMs. Patients having positive FM at admission were not different according to severity (ICU referral at admission) or in-hospital mortality. When analyzing FMs positivity according to the first 9 days of hospitalization, we figure out that 37% of the patients had positive FMs at least once during stay in hospital. These patients had increased in-hospital mortality (p=0.001). To confirm this result, we performed non-adjusted Kaplan–Meier curves for in-hospital mortality according to FMs positivity during hospitalization and we demonstrated a statistically significant differences for in-hospital mortality (HR=1.48, 95% CI 1.25−1.76, p< 0.001).

    Conclusion(s): Increase in during COVID-19 hospitalization could represent a reliable and helpful tool to predict worsening and mortality.
