    Background: All patients with hemophilia B without factor IX inhibitors could benefit from extended half-life (EHL) factor IX (FIX) products; certain groups may benefit substantially, for example, patients with target joints or arthropathy, patients experiencing frequent bleeds, adolescents, athletes and those with poor venous access or treatment adherence. rIX-FP has demonstrated efficacy in prophylactic regimens up to every 21 days in patients with hemophilia B. In order to optimize patient outcomes, it’s important to ensure that transitioning to longer dosing intervals is conducted as per label recommendations.

    Aims: To provide a practical guide for healthcare professionals in managing the appropriate transition to rIX-FP, including switching, surgical management, and on-demand use.

    Methods: Clinical case studies will be provided by the authors following real-life experiences in their clinical settings.

    Results: The following clinical case studies on appropriate transitioning to rIX-FP will be presented. Appropriate and successful transitioning to rIX-FP resulting in the following outcomes: less frequent dosing, improved quality of life, and higher physical activity. The use of rIX-FP in surgical settings, resulting in improved recovery times and patient-reported outcomes due to reduced hospital visits. Using rIX-FP in emergency/on-demand use, demonstrating the successful management of spontaneous bleeds. Appropriate strategies for managing switching to rIX-FP, including using pharmacokinetic modeling to optimize dosing.

    Conclusion(s): Appropriate switching to rIX-FP, as per label guidance, allows patients to be safely transitioned to a 21-day dosing regimen. Following switching to rIX-FP, patients can obtain efficient bleed protection and a lower disease burden, which could lead to improvements in quality of life. Reduced consumption of factor replacement product may allow more effective management of surgical hemostasis. The higher and prolonged trough levels provided by rIX-FP may enable on-demand users to manage bleeding episodes with more confidence.
