    PB1521 - Evaluating haemostatic changes during pregnancy with global coagulation assays – A prospective observation study

    Background: The pregnancy state results in changes to coagulation profiles which can predispose to both bleeding and thrombotic complications. While traditional screening coagulation assays are not sensitive for detecting such haemostatic changes, global coagulation assays (GCA) such as clot waveform analysis (CWA) and thrombin generation assays (TGA) may be more suited to detect such changes.

    Aims: We aim to study the changes in aPTT-CWA and TGA in pregnant females throughout the different phases of pregnancy.

    Methods: Plasma samples were collected from patients during the three trimesters of pregnancy as well as immediate post-partum, and at least 6 weeks post-delivery. Standard coagulation assays were performed. aPTT-CWA parameters were generated using a CS2500 coagulometer (Sysmex). TGA was performed using the Calibrated Automated Thrombogram (CAT).

    Results: 50 females were recruited for the studies. (Table 1). Prothrombin time (PT) and activated partial thromboplastin time (aPTT) parameters were within the normal reference ranges throughout the phases of pregnancy. Changes in coagulation profile were seen early in the first trimester with the elevation of D-Dimer and fibrinogen and decrease in Protein S. GCA parameters changes during the 3 trimesters were seen with an increasing mean of all aPTT-CWA parameters (min1, min2 and max2, p< 0.001). (Figure 1) A decline was seen in the immediate postpartum period with return to normal range at baseline of 6 weeks postpartum. Endogenous thrombin potential (ETP) on CAT was elevated during all phases of pregnancy and peaked during the second trimester.

    Conclusion(s): Changes in haemostatic parameters happens early in pregnancy during the first trimester. These changes were demonstrated well in global coagulation assays especially aPTT-CWA. Such assays may serve as potential assays to monitor coagulation status during pregnancy.
