Background: Hemophilia management aims to prevent bleeding and preserve joint function. Changes in patients’ joint health may influence physicians' decisions to adjust hemophilia treatment. The Hemophilia Joint Health Score (HJHS) and Hemophilia Early Arthropathy Detection with Ultrasound (HEAD-US) score assess joint health but are not routinely used in clinical practice.

Aims: To evaluate if systematic joint examination with HJHS and HEAD-US impacts treatment management decisions in patients with hemophilia A (PwHA).

Methods: A-MOVE (NCT04133883) was a 12-month prospective, descriptive, low-interventional multicentre study, which enrolled PwHA (all severities, aged between 6–40 years) treated prophylactically or on demand with standard or extended half-life FVIII replacement therapy. At each study visit (baseline, 6 and 12 months), HJHS and HEAD-US were measured. Changes in patients’ management (including treatment regimen and product, physical therapy, healthcare professional contact, pain and anti inflammatory medication, intra-articular injections, surgical intervention etc.) and whether this was impacted by HJHS/HEAD-US scores were recorded. Secondary objectives included evaluation of joint health, pain and functional activities.

Results: Eighty-six patients from 20 sites in France were included in this analysis (Table 1); 68 had HJHS/HEAD-US assessments at 12 months (66 patients completed the study). Over the entire evaluation period, 24.4% (n=21/86) of patients experienced an impact on their hemophilia management due to HJHS or HEAD-US scores (Table 2). In 52.4% (n=11/21) of these patients, hemophilia management decisions were impacted by functional/physical exams (HJHS); 95.2% (n=20/21) were impacted by ultrasound examination (HEAD-US). Both assessments contributed to change in management decisions in 47.6% of patients (n=10/21).

Conclusion(s): A-MOVE data show that systematic joint assessments, through functional/physical examination and ultrasound, impact hemophilia treatment management decisions in PwHA. HEAD-US evaluations may be more sensitive than HJHS in detecting joint changes, which may help physicians decide whether to adjust individual treatment management earlier.
