    Background: The issue of pregnancy management in women who have had thrombosis is a difficult task, since women with a previous episode of venous thromboembolic events (VTE) have a 3-4 times higher risk of VTE during subsequent pregnancies.

    Aims: in order to improve the existing methods of prevention, we have studied other risk factors, in particular polymorphisms of the PAI-I, F2 Thr165Met genes as possible new candidates for thrombosis risk factors in pregnant women.

    Methods: we examined 50 women with history of venous and arterial thrombosis and obstetric complications and 40 patients with the physiological course of pregnancy (control group).

    Results: 24 (48%) out of 50 women developed VTE during pregnancy. After thrombosis risk assessment at the first trimester of pregnancy, only 7 (29.2%) were considered as candidates for thromboprophylaxis. The remaining 17 women who subsequently had thrombosis did not receive anticoagulants. Most women were not tested for thrombophilia, despite having a history of pregnancy complications. The genetic high-risk thrombophilia was found in 5 (20.8%) of 24 women with thrombosis during pregnancy (3 – homozygous forms of FV Leiden mutation, 1 – protein C and 1 – protein S and AT deficiency). Circulation of various types of aPLa was found in 13 (54.2%) of 24 women. Homozygous form of PAI-I 4G/4G gene polymorphism (45.8%) and prothrombin F2 Thr165Met (20.8%) (fig. 1) were significantly more common in women with thrombosis and obstetric complications compared with the control group (p < 0.001 and p=0.023, respectively; OR 6.744, 95% CI: 1.195-38.056).

    Conclusion(s): After the exclusion of high-risk thrombophilia, there are a number of polymorphisms that may be a new candidate for thrombosis risk factor in pregnant women. More than 50% of thrombosis in our work were associated with the homozygous genotype of the type 1 plasminogen inhibitor 4G/4G (p < 0.001) and prothrombin F2 Thr165Met (p < 0.001).
