Background: Retinal vein occlusion (RVO) and superior Ophthalmic Vein Thrombosis (SOVT) are rare diseases in the pediatric population, however, the ophthalmic and neurologic morbidity are significant. Scarce data is available regarding underlying etiology as well as management in the pediatric population and relays mainly on case reports.

Aims: Our aim was to share our single-center experience with ocular venous occlusion in pediatric patients as well as review the current literature and discuss the etiologies, clinical presentation, workup and treatment modalities.

Methods: Our institutional computerized database was searched, extracting all pediatric patients with documented RVO and SOVT treated in our center over the past 10 years. The study was approved by the Institutional Review Boards.

Results: Our cohort was comprised of four patients. Two patients were followed and treated for RVO and additional thrombi, one patient with isolated central RVO and one patient with SOVT with progression to the cavernous sinus following sinusitis. All patients received anticoagulation besides one, who improved rapidly following diagnosis. Patients’ diagnoses, treatments and outcomes are presented in Table 1.

Conclusion(s): In children with either RVO or SOVT, thrombophilia workup is suggested. In patients with thrombophilic risk factors or patients with additional thrombi we highly recommend initiating anticoagulation therapy. Due to the rarity of these clinical entities in the pediatric population, we recommend a case-by-case anticoagulation management. There is a need for more research in order to determine the optimal management strategy.
