Background: There is a lack of epidemiological data regarding pediatric venous thromboembolism (VTE) in Brazil.

Aims: To establish a registry (Adventh-Pediatric Registry) of symptomatic VTE in children and adolescents admitted to 3 tertiary hospitals in Campinas, SP, Brazil: pediatric ward and ICU of Hospital de Clínicas- UNICAMP; nursery with ICU in CAISM-UNICAMP; pediatric oncohematologic hospital, Boldrini.

Methods: Multicenter prospective observational study of VTE of any site, symptomatic associated with central venous catheter (CVC), spontaneous or incidental. All patients were evaluated daily for the presence of VTE by medical contact or record data. Only those with a diagnosis established by an objective image method were included. All patients with CVC insertion were observed daily and doppler ultrasound (US) was performed if any sign suggested thrombosis.

Results: Between September 2018 to January 2022, from 5040 hospitalizations, 39 cases of symptomatic VTE were diagnosed, corresponding to an incidence of 0.0076%, (76 thrombosis for every 10000 pediatric hospitalizations). The affected sites were the upper limbs (n = 20 / 51.3%), lower limbs (n = 17 / 43.6%), abdome (splenic vein) (n = 1 / 2.5%), and upper limb + lungs (n = 1 / 2.5%). Most VTE cases were associated with CVC (27 cases - 70%). The median time between CVC insertion and VTE diagnosis was 18 days (0 - 671 days). Risk factors were present in 95% of the children. Most cases were diagnosed by US (n = 34 / 91%). Anticoagulant therapy was used in 31 (80%) patients.

Conclusion(s): Preliminary data from this first Brazilian pediatric VTE registry without active searching indicates that its prevalence in hospitalized children is similar to that described in other populations, and is largely associated with the presence of CVC.
