Background: Primary immune thrombocytopenia (ITP) is an autoimmune disease characterized by decreased platelet count resulting from immune-mediated platelet destruction and reduced platelet production. Thrombopoietin (TPO) being the major regulator of platelet production is expected to be involved in the immune regulation of T cell subsets in ITP. However, it is not very clear.

Aims: The aim of this study was to measure serum TPO levels and investigate whether serum TPO levels were correlated with platelet count and proportions of T cell subsets in children with ITP.

Methods: Pediatric patients diagnosed with ITP were enrolled in this study at Beijing Children’s Hospital. Serum TPO levels were measured by ELISA. The proportions of T cell subsets (CD4+ T, CD8+ T, Th1, Th2, Th17, and Tregs) were investigated by flow cytometry.

Results: In total, 110 patients with newly diagnosed ITP (NITP), 43 patients with persistent ITP (NITP), and 58 patients with chronic ITP (CITP) were enrolled in this study. On Kruskal-Wallis test, the levels of serum TPO were significantly different among the three groups (P=0.003) while that of platelet counts were not. In all patients, a reverse correlation was found between serum TPO levels and total T cells (r=-0.157, P=0.023). Unexpectedly, the relationship between serum TPO levels and proportions of T cell subsets was not observed.

Conclusion(s): The preliminary results suggest that ITP patients presented with differences in serum TPO levels, while serum TPO levels do not appear to play a critical role in differentiation of T cell subsets.
