    PB1169 - Evaluating the Clinical Utility of Prothrombin Time-derived Fibrinogen Method

    Background: Clauss method (Reference range: 238-498 mg/dL) and PT-derived method (Reference range: 276-471 mg/dL) are mostly used to determine the fibrinogen concentration. And both assays are peer groups in the proficiency CAP test (CGL etc.). Clauss method is suggested by CLSI H30, with specific reagents and high cost, while PT-derived method is easy, economical and able to determine with PT at the same time.

    Aims: In routine coagulation tests, PT and APTT are used, but rarely determine fibrinogen concentrations with the PT-derived method. And when fibrinogen determination is needed, Clauss method is recommended in most medical centers, while it may not generate the report instantly. This research is to evaluate the clinical utility of PT-derived fibrinogen method.

    Methods: All specimens were detected with an ACL TOP750 analyzer with required fibrinogen reagent (IL Q.F.A Thrombin) and PT reagent (IL RecombiPlastin 2G). And correlation between Clauss and PT‐derived methods were analyzed. Specimens were randomly collected in 48 days of PT data with PT-derived fibrinogen for further investigation.

    Results: The correlation between Clauss and PT‐derived methods was high (R2 = 0.972 > 0.95, n=100;Figure1). We use the Clauss method fibrinogen < 100 mg/dL (critical value) as reference to determine that the critical value of the PT‐derived method is < 150 mg/dL. A total 24,545 PT-derived fibrinogen results were included in final analysis. There are 225 (1%, 225/24545) below critical value. Among the 225 low PT‐derived fibrinogen data, 189 (84%) were clinically followed, 25 (11%) not followed and 11 (5%) clot samples. It shows most low fibrinogen patients can be followed by routine coagulation tests (Clauss method), however there is a chance (0.1%) to miss out(Figure2).

    Conclusion(s): This study showed that determination of PT-derived fibrinogen concentration with PT simultaneously is an effective and rapid screening tool for low fibrinogen patients, and can also be an alarm for clot samples.
