    PB1106 - A 2-min 30 sec at 4500g centrifugation or a 11-min at 2500g are comparable on 13 coagulation assays

    Background: The French Group of Studies on Hemostasis and Thrombosis (GEHT) recommends a centrifugation of 10 min at an acceleration between 2000 to 2500g. To our knowledge rapid centrifugation was never compared to standard centrifugation with a large cohort and on of patients on more than 10 parameters.

    Aims: This study aims to assess, for each tested parameter, if results of the haemostasis tests derived from a 2min 30sec and from a 11-min centrifugation time are identical.

    Methods: All adult patients in emergency departments for whom a haemostasis test was prescribed were included in the study. According to French law, oral consent was obtained. For each patient, two tubes were collected into BD Vacutainer® 9NC 0.109M, 2.7mL. These tubes were centrifuged simultaneously: one during 2 min 30 sec at 4500g and the other one during 11 min at 2500g. Residual platelets after centrifugation were counted in the 40 first patients. The exclusion criteria were a haemolyzed or insufficient plasma. Then, simultaneously, 13 tests were performed on Atellica COAG 360®, Siemens. The analytical mismatch is defined as a difference in result greater than 4.24 x the maximal tolerated intra-technique standard deviation.

    Results: A total of 201 patients were included. Residual platelets counts were below 10G/L. After exclusion of haemolyzed or insufficient plasma, results concordance between the two centrifugation protocols were evaluated in 184 patients and 52 patients for factor VIII, factor IX, factor XI and von Willebrand factor activities. For the 13 parameters studied, 90.2%-100% are concordant. The few analytical mismatches had no clinical impact (table1).

    Conclusion(s): Unlike previous studies, this one focused on a large number of patients and a large range of haemostasis parameters. Shortening the centrifugation time has no impact on the interpretation of coagulation test results and may help in critical situations.
