    PB0971 - Incidence of cancer-associated thrombosis in ambulatory cancer patients not receiving thromboprophylaxis, A prospective cohort study in Asian population

    Background: To date, there is limited data regarding the incidence of cancer-associated thrombosis (CAT) among Asian patients who do not receive thromboprophylaxis.

    Aims: To investigate the incidence of CAT in Asian population.

    Methods: This was a prospective cohort study conducted at a tertiary medical center from June 2020 to December 2021. We enrolled cancer patients who were 18 years old or older, planned to receive the first cycle of chemotherapy, and had an Eastern Cooperative Oncology Group (ECOG) performance status of not greater than 1. All patients were followed for 6 months after the first cycle of chemotherapy. The primary outcome was the incidence of venous thrombosis and mortality.

    Results: A total of 680 cancerpatients in the study. At 6 months follow up, VTE occurred in 37 patients (5.44%). The median time to the first thrombosis was 2.08 months (interquartile range 0.30-3.38). Out of the 680 patients, 68 (10%) died. In multivariable analysis, cholangiocarcinoma (HR 7.83, 95%CI 2.88-21.67, p< 0.001) and a Khorana score ≥3 (HR 10.90, 95%CI 1.81-65.71, p=0.009) were significantly associated with VTE. Patients with a higher risk of thrombosis (Khorana score ≥3) developed VTE 11.84% of the time, while those with scores 1-2 and score 0 developed VTE 5.58% and 1.45%, respectively (figure 1). The C-statistic score was 0.51 when using a Khorana score cut-off of ≥2 and 0.57 when using a cut-off of ≥3. Cancer patients who had VTE (HR 5.36, 95%CI 1.01-28.55, p=0.049), Stage 4 cancer (HR 7.84, 95%CI 1.64-37.59, p=0.010), or a Khorana score ≥3 (HR 5.63, 95%CI 1.39-22.80, p=0.015) had a higher risk of death.

    Conclusion(s): A significant number of ambulatory cancer patients who did not receive thromboprophylaxis developed venous thrombosis. Asian patients with Khorana score cut-off of ≥3 should be considered thromboprophylaxis.
