    Background: TiC-Onco score is a VTE risk score (RS) developed in oncology patients receiving chemotherapy in ambulatory setting that incorporates clinical and genomic variables.

    Aims: To evaluate predictive capability of TiC-Onco score in the long-term follow-up in the prospective cohort ONCOTHROMB12-01, at different timepoints (12 and 18 months) and compare with Khorana RS.

    Methods: 391 consecutive patients recruited from the ONCOTHROMB12-01 observational, prospective, multicentre and non-interventional study (NCT03114618) conducted in 7 Spanish hospitals between March 2013-May 2016 were analyzed. The study was approved by local IRB and signed informed consent was obtained from each patient. Main inclusion criteria: colorectal, oesophago-gastric, lung, or pancreatic cancer receiving systemic outpatient chemotherapy and not receiving thromboprophylaxis. TiC-Onco RS variables (5): type of tumor, clinical stage, BMI, family history of VTE and the analysis for 4 single nucleotide polymorphisms (SNPs) in FV, FXII and serpin 10. Predictive capability was assessed calculating AUC.

    Results: VTE incidence in the cohort was 22.7% (89 patients). The extended follow-up (from month 7 to 18) identified up to 20 new cases of VTE (22.4%). The majority of the events were incidental 68 (64.1%). Most common locations were pulmonary embolism 35 (33.0%) and visceral vein thrombosis 34 (32.0%). TiC-Onco (5 variables) performed better than Khorana RS in the long-term evaluation of VTE risk. Table 1 summarizes predictive capability at different time-points.

    Conclusion(s): TiC-Onco RS is the first clinic and genomic model that predicts VTE risk in a long-term follow up for ambulatory cancer patients. The addition of genomic variables that remain unalterable to RS models is key for a dynamic approach to the evaluation of cancer associated thrombosis.
