    Background: Venous thromboembolism (VTE) in cancer patients complicates the clinical course and has been suggested to add to the psychological burden of disease.

    Aims: To investigate the association between cancer-associated VTE and subsequent depression in patients with hematological cancer.

    Methods: We conducted a Danish nationwide, population-based cohort study using administrative and medical registries. A cohort of 1,069 patients with hematological cancer and incident VTE during 1995-2020 and a comparison cohort (n=4,804) of hematological cancer patients without VTE, matched according to birth-year, sex, cancer type, and cancer diagnosis-year, were identified (Table 1). Depression was defined as hospital diagnosis of depression or prescription for antidepressants. Patients were followed for up to five years from VTE diagnosis/index date. Absolute risks were estimated using cumulative incidence functions, treating death as competing event. Incidence rates (IRs) were calculated and hazard ratios (HRs) with 95% confidence intervals (CIs) were computed in Cox proportional hazards models, using the comparison cohort as reference.

    Results: Depression was observed in 175 hematological cancer patients with VTE and in 700 without VTE. The absolute risk of depression was 8.8% (95%CI, 7.2-10.6), 13.8% (95%CI, 11.8-16.0), and 16.8% (95%CI, 14.6-19.2) after 1-, 3-, and 5-years of follow-up in the VTE cohort, compared to 5.1% (95%CI, 4.5-5.8), 11.4% (95%CI, 10.5-12.3), and 15.3% (95%CI, 14.2-16.3) in the comparison cohort (Fig. 1). The corresponding IRs of depression were 114.1 (95%CI, 91.0-137.1), 73.9 (95%CI, 62.0-85.9), and 62.0 (95%CI, 52.8-71.2) per 1,000 person-years, compared to 55.8 (95%CI, 48.9-62.8), 48.0 (95%CI, 43.9-52.0), and 43.9 (95% CI, 40.6-47.1). After adjusting for matching variables and comorbidities, VTE was associated with depression, with HRs of 2.15 (95%CI, 1.65-2.79), 1.52 (95%CI, 1.24–1.86), and 1.44 (95%CI, 1.20–1.74) after 1-, 3-, and 5-years of follow-up.

    Conclusion(s): In patients with hematological cancer, VTE was associated with increased risk of subsequent depression, thereby further adding to the burden of cancer-associated VTE.
