Background: Extracorporeal membrane oxygenation (ECMO) is used in the case of acute respiratory failure and open heart surgery. A case to require ECMO exchange is reported after the pressure of the inlet of ECMO increased, but the cause is unknown. To elucidate the cause, we tested it under the simulated extracorporeal circulation condition. Desialylated platelets (d-plt) gradually increased during the circulation in a time-dependent manner. However, the function of d-plt isn’t clear.

Aims: The aim of this study was to elucidate the function of d-plt increased under the simulated extracorporeal circulation condition.

Methods: Platelet rich plasma (PRP) were treated with neuraminidase (Nase) after freshly drawn whole blood from healthy volunteers was centrifuged to isolate PRP. The function of d-plt was assessed by measuring platelet adhesion and platelet aggregation. Furthermore, platelet activation markers were also measured by flow cytometry.

Results: There were no significant differences between d-plt and control platelets untreated with Nase (c-plt) in the rate of the adhesion to collagen or fibrinogen. On the other hand, platelet aggregation induced by ADP or collagen was enhanced in d-plt compared to c-plt. The number of d-plt with phosphatidylserine-exposure, glycoprotein IIb-activation or P-selectin-expression were significantly increased rather than those of c-plt, respectively. However, the microparticle release ability wasn’t different between them. Furthermore, the prothrombin time and activated partial thromboplastin time were not primarily related to coagulation activity with the addition of d-plt.

Conclusion(s): These data suggest that monitoring of d-plt may help to early detect a trouble of ECMO in clinical settings.
