Background: Antiphospholipid syndrome is an autoimmune, proinflammatory, prothrombotic, and systemic disorder characterized by arterial or venous thrombosis, or both, recurrent fetal loss, and persistently elevated titers of antiphospholipid antibodies.

Aims: Investigate the main antibodies present in cases of antiphospholipid syndrome with acute thrombosis

Methods: Adult patients with antiphospholipid syndrome who have presented acute arterial and venous thrombotic manifestations are studied; All of them undergo an antibody panel that includes the determination of antinuclear antibodies, anti-DNA, anti-SM, lupus anticoagulant, anti-B2 glycoprotein, IgM and IgG anticardiolipin, and IgM and IgG antiphospholipid

Results: Twenty cases with a confirmed diagnosis of the antiphospholipid syndrome are analyzed, the most affected gender was female 85%, and the average age is 31 years; all presented thrombotic events, highlighting deep vein thrombosis (65%), recurrent abortions (20%), pulmonary embolism (10%) and multiple organ failure due to micro thrombosis associated with the catastrophic variety (5%). Fourteen cases were primary, five cases were secondary (four associated with lupus and one with Sjögren's syndrome) and 1 case was of the catastrophic variety. Regarding the presence of autoantibodies, 100% had IgM anticardiolipin antibodies, 90% had lupus anticoagulant, 85% IgM antiphospholipid antibodies, 50% anti B2 glycoprotein, 50% IgG anticardiolipin antibodies, 40% IgG antiphospholipid antibodies. Other documented autoantibodies were antinuclear 35%, anti-DNA 20%, and anti-SM 20%.

Conclusion(s): In this series of cases with acute thrombotic manifestations due to antiphospholipid syndrome, IgM anticardiolipin antibodies and lupus anticoagulant are the most prevalent.
