Background: Platelet products (PLT) are a critical aspect of hemostatic resuscitation. The majority of PLT in the United States (US) are collected via apheresis. It is currently unclear how the combinations of collection device and resuspension medium impact platelet hemostatic quality.

Aims: We evaluated single-donor apheresis platelet units collected using different platforms to determine resulting differences in in vitro hemostatic efficacy.

Methods: PLT were collected (N=10 each group) via the MCS®+ 9000 (Haemonetics; “MCS”), Trima Accel® 7 (Terumo; “Trima”) into Isoplate (“TI”) or plasma (“TP”), and the Amicus Cell Separator (Fresenius Kabi, “Amicus”) into InterSol (“AI”) or plasma (“AP”). Units were sampled 1hr post-collection. Calculated yield determined by platelet count, biochemistry (pH, lactate, glucose, sodium (Na), and potassium (K)), hemostatic function using aggregometry, rotational viscoelastometry (ROTEM), and thrombin generation, and microparticle content using flow cytometry were measured. Statistical comparisons were made using unpaired t-tests (alpha=0.05).

Results: Mean calculated yields were 3.0-3.5x10^11 platelets/unit. Biochemical analysis showed elevated glucose in MCS compared to TP (p < 0.001) and AP (p < 0.001), with no other differences in pH, lactate, Na, or K. When compared to MCS and TP, AP demonstrated decreased aggregometry area under the curve (AUC) when activated with dual agonists, partnered with decreased ROTEM maximum clot formation (MCF) (Figure 1). Similarly, AI had decreased AUC with dual agonists, and had lower endogenous thrombin potential (ETP) in contrast to TI, despite an elevated microparticle presence (Figure 2).

Conclusion(s): Decreased AUC in Amicus PLT, coupled with smaller MCF values suggest PLT collected via Amicus may be less likely to form an effective clot despite increased microparticles. These findings indicate substantial differences between apheresis collection methods and warrant further investigation with future emphasis how function changes over storage time and clinical relevance in bleeding patients.
