    Background: Coronary artery disease (CAD) is a leading cause of death worldwide. Genetic variability accounts for ≈30% of CAD predisposition and was mainly investigated in populations of European and Asian ancestry.

    Aims: To investigate the association of selected gene variants with early-onset CAD in Iranians.

    Methods: We performed a case-control study of young patients (females < 55/males < 45 years) undergoing coronary angiography at the Tehran Heart Centre (Iran) in 2004-2011. Cases had coronary artery stenosis≥50% in at least one main artery and no history of myocardial infarction. Controls were age-, sex-matched individuals without CAD. Twenty-five variants, 14 SNPs previously associated with CAD and 11 in hemostatic genes, were genotyped using a custom TaqMan®OpenArray™ panel. Genotyping quality control criteria were SNP-call rate < 95% and differential cases/controls missingness (p < 0.00001). Association was assessed using age, sex-adjusted logistic regression, stratifying by ethnicity.

    Results: We genotyped 887 cases and 950 controls. After quality control, 697 cases, 911 controls (Table 1) and 24 variants (Table 2) remained for analysis. Of 13 previously associated SNPs, 3 were confirmed as associated in Iranians (SORT1,HCG27-HLA-C,CDKN2A-CDKN2B), 5 showed ORs consistent with previous studies, whereas 5 were not confirmed as risk factors in this population. Of 11 SNPs in hemostasis genes, we identified 1 risk (FV) and 2 protective factors (TSPAN15,rs2289252 FXI) for CAD.
    Ethnicity was a strong risk/protective factor at stratification analysis. The six aforementioned associations were mainly driven by Fars (HCG27,CDKN2,TSPAN15,FXI), Tork (SORT1), and other ethnicities (FV). Three additional SNPs emerged in Fars (LPA,rs35335161-VWF) and Tork (MIA3). A fourth SNP (CXCL12) showed opposite effects, increasing and decreasing CAD risk in Fars and other ethnicities, respectively.

    Conclusion(s): Ten SNPs, of which three previously implicated in venous but not arterial thrombosis, were associated with early-onset CAD in Iranians. Ethnicity specific associations underlie the importance of studying the genetic causes of CAD in genetically diverse, underrepresented populations.
