Background: COVID-19 is associated with severe endotheliopathy, thromboinflammation and immunothrombosis. These processes lead to excessive release of von Willebrand factor (vWF) multimers from Weibel Palade bodies, which can affect the activity of metalloproteinase ADAMTS-13 and the ADAMTS-13/vWF axis. Our previous work showed changes in non-pregnant women with severe Covid-19. Our task was to study the peculiarities of the functioning of the axis in pregnant women with COVID-19 and convalescents.

Aims: to study the functioning of the ADAMTS-13/vWF axis in pregnant women with Covid-19 and after infection during pregnancy.

Methods: A case–control study was conducted, which included pregnant women in the 3rd trimester of pregnancy (n=105). The patients were divided into 3 groups, group I included 45 women with coronavirus infection during pregnancy, group II – 45 women in the acute phase of infection during pregnancy, group III - 15 healthy pregnant women. The content of vWF and ADAMTS-13 was determined in all patients.

Results: The concentration of vWF (vWF:Ag, IU/ml) in the acute period of the disease in pregnant women with COVID-19 was significantly increased compared to the control group. The level of ADAMTS13 in pregnant women after Covid-19 did not differ from the control group, while the concentration of vWF was much higher in 67% (30/45). The ratio of ADAMTS-13/vWF was reduced and significantly differed in pregnant patients during the acute period of the disease, compared with the control group (p= 0.0005) (table 1).

Conclusion(s): Our results show that endotheliopathy is significantly manifested in pregnant women with Covid-19 and persists for several months after COVID-19 in some patients. As a result, plasma levels of vWF significantly increase, and ADAMTS-13 levels decrease. Monitoring of these biomarkers can be used to determine risk, diagnose, monitor the effectiveness of treatment and predict the outcome in pregnant women and possibly improve therapy.
