Background: General health is not only directly but also indirectly affected by coagulation disorders. Family is a socio-cultural system that exerts remarkable influence on children and mother’s health. Hemophilia cause psychosocial challenges for family members specifically mothers like stigmatization and impression on their decisions such as pregnancy complications, as the possibility of affected sons and carrier daughters. Also, due to main concerns like unexpected bleedings and children’s behaviors, parenting style is crucial for children’s psychophysiological health.

Aims: Current study explores the experiences of mothers with severe hemophiliac children through family interactions and their applied parenting styles in Iranian culture.

Methods: This phenomenological study used an inductive thematic analytical approach for analyzing collected data by semi-structured in-depth interview. To adhere cultural concerns, interviews were implemented by a female psychologist. MAX.QDA software package 2020 applied for data arrangement and the participants were invited from Mofid Comprehensive Care Center for Children with Hemophilia, Tehran, Iran.

Results: Through 15 interviews, 6 themes consisting 27 subthemes in family interactions were observed which embrace inter-personal conflicts mostly in marriage and pregnancy, family disruption and apportion of duties, non-differentiation between mothers and children and spouses, supportiveness of family members and lack of medical knowledge. Furthermore, 4 parenting styles were extracted including authoritative, authoritarian, permissive and snowplow.

Conclusion(s): Under the influence of cultural gender stereotypes, mothers demonstrated sense of loneliness which is related to their non-assertiveness to contribute desires with their husbands and perceive them not responsive enough. Disruption in family interactions generates parenting inconsistency which causes mental splitting in children that permissive father is good and domineering mother is bad. Despite all of pregnancy complications, abortion as a medical option arises feeling of guilt in mothers and impacts on their psychological health. Proper strategy to how inform daughters about their possible carriership of disease is a dilemma for families and medical staff.
