Background: Edoxaban is the last direct oral anticoaugulant licensed for venous thromboembolism (VTE) in Europe. To date, at variance with dabigatran, rivaroxaban and apixaban, real-life data on the use of edoxaban are few.

Aims: To describe the use of edoxaban in a real-life setting and to assess the efficacy and safety of edoxaban for therapy of VTE in real-life.

Methods: This is a monocentric retrospective cohort study, including adult patients referred to our Milan center for a VTE and treated with edoxaban from 2015 (data-first marketing in Italy) to December 2021. Previous treatment with low molecular weight heparin or a vitamin K antagonist lasted more than 3 months with edoxaban was considered an exclusion criterion. All enrolled patients have been followed-up until a thrombotic or bleeding event, edoxaban discontinuation, or study end in December 2022 (whichever came first).

Results: In the study period 150 out of 173 eligible patients have been included (Figure) and followed for a total follow-up of 168 years. Median age of the study population was 52(IQR 42-65) years and 88(59%) were males. Most patients (121, 81%) were treated for a deep vein thrombosis and/or a pulmonary embolism while 29(19%) for a VTE at unusual sites (9 cerebral, 7 splanchnic, 9 upper extremities and 4 superficial vein thrombosis).A complete recanalization was obtained in 70 (47%) patients, 48 (32%) reached a partial recanalization, 3 did not recanalize, and 29 (19%) were not evaluated yet at 1 year from edoxaban initiation. During a mean follow-up time of 13.5 months only one superficial vein thrombosis, 2 major and 11 minor bleedings were observed (Table).

Conclusion(s): In our cohort of patients, edoxaban showed a good real-life performance for VTE treatment at common sites as well as in the 19% of patients with VTE at unusual sites.
