    PB1086 - Time in therapeutic range after an event: where we started from and where are we going?

    Background: Vitamin K antagonists are the anticoagulation therapy available for the primary and secondary prevention of venous and arterial thrombosis. The strict control of anticoagulant therapy through the INR reduces the risk of thrombosis or bleeding. INR control is suboptimal when time in therapeutic range (TTR) is < 60%.

    Aims: The main objective is to observe the differences in the TTR, after the presence of a thrombotic or hemorrhagic event.

    Methods: First, a database of 291 patients was created. The patients started anticoagulation from November 2019 to April 2020 and were followed up for one year. Secondly, the patients data were collected: age, gender, reason for anticoagulation, presence of hemorrhagic or thrombotic event and TTR. Third, the TTR of anticoagulated patients was observed according to gender, age, reason for anticoagulation and event.

    Results: In this study, the 57.7% were men and 42.3% women. The mean age was 74.97 years. The main reasons for anticoagulation were Atrial Fibrillation (AF) in 61.51%, Deep Vein Thrombosis (DVT) in 14.43%, Pulmonary Thromboembolism (PTE) in 11% and 13.06% for other reasons. The 92,17% of the patients who started anticoagulation due to AF, were older than 60 years. AF and PTE predominated in men, however, DVT in women.
    The 52.2% had a suboptimal TTR. The 48.6% of patients with AF and 40.6% with PTE had a TTR < 60, in opposite to patients with DVT, which was 66.66%. Patients < 60 years and women had worse TTR.
    During the year of follow-up, 12.71% of the patients presented a thrombotic event (TABLE1) and 24.39% a hemorrhagic event (TABLE2). After a thrombotic event, 91.66% presented a TTR >60 and after a hemorrhagic event, 81.70%.

    Conclusion(s): The factors associated with a low TTR are: female sex, DVT and age less than 60 years. After a hemorrhagic or thrombotic event, anticoagulated patients improve the TTR.
