    PB0696 - Management of bleeding in people with hemophilia A on prophylaxis with emicizumab in emergency situations: Recommendations from a Spanish expert panel

    Background: Hemophilia A (HA) is a rare congenital coagulopathy resulting in hemorrhagic events that need to be treated in the emergency department quite often. Prophylaxis with emicizumab, a FVIII mimetic biespecific monoclonal antibody in people with hemophilia A, makes it necessary to update the recommendations for attending bleeding events in this setting.

    Aims: The aim of this communication is to provide expert recommendations on the management of bleeding complications in people with hemophilia A treated with emicizumab in the emergency room.

    Methods: A Spanish multidisciplinary expert panel, constituted by hematologists and emergency physician, organized the current state of the art and designed a clinical algorithm to face this topic

    Results: The algorithm is divided in three parts: 1) Patient identification, to help physicians to classify patients according to the presence/absence of inhibitors. 2) Clinical management of bleeds and 3) laboratory tests. Parts 2 and 3 are based on the previous assessment with the aim to guide the physician with recommendations for the treatment of the bleeding event (Figure 1) and recommended laboratory tests (Figure 2).

    Conclusion(s): This algorithm can expedite the emergency treatment of breakthrough bleeding in an emergency room in people with HA on prophylaxis with emicizumab. The algorithm will be shared with hematologists who are not familiarized with the management of patients with hemophilia as well as to emergency physicians.
