PB0003 - Clinical Outcomes and Associated Factors in Patients Who Underwent Successful Percutaneous Coronary Intervention: A 5-Year Multicenter Retrospective Study from a Developing Country

Background: Percutaneous coronary intervention (PCI) became a standard management modality to improve patients’ quality of life and reduce mortality when combined with pharmacologic therapy in myocardial infarction patients. However, the PCI service was not well-practiced in Ethiopian health facilities and also poor outcomes had previously been reported.

Aims: To evaluate the clinical outcomes and associated factors in patients who underwent successful PCI.

Methods: We conducted a five-year retrospective study by including 241MI patients who underwent successful PCI. The outcome measurements were in-hospital mortality, major adverse cerebrovascular and cardio-vascular events (MACCEs), post-procedural complications and survival rate. Data were analyzed using Statistical Package for Social Sciences version 25. Cox proportional-hazard regression models were used to assess predictors of in-hospital mortality, hazard ratios, and 95% confidence intervals were presented. We used the Kaplan-Meier analysis to assess overall survival rates and median survival time.

Results: Out of 241 participants, the majority were males 194 (80.5%) and the mean age was 57.2 ± 10.6 years. One twenty-three (51%) patients received pharmaco-invasive PCI and drug-eluting stent (75.1%) implantation as a reperfusion method. In-hospital mortality and non-fatal MACCE rates were 3.7% and 24.1%, respectively. The predictors for in-hospital mortality were being female (adjusted hazard ratio (AHR)=9.10, 95% CI:1.04-79.34, P=0.046), pre-procedural obesity (AHR=6.85, 95% CI: 1.13-41.46, P=0.036), previous MI (AHR=11.89, 95% CI:1.99-71.07, P=0.00, chronic heart failure (AHR=9.53, 95% CI: 1.19-76.43, P=0.034) and having a previous history of stroke (AHR=25.85, 95% CI: 2.03-329.63, P=0.012). The overall estimated 1-year survival rate of MI patients who underwent successful PCI was 96.3%.

Conclusion(s): Pharmaco-invasive PCI was the most commonly conducted intervention in MI patients. The overall in-hospital mortality rate was low and about a quarter of patients developed non-fatal MACCEs. The one-year survival rate was high.
