PB1291 - Joint range of motion findings among female patients with hemophilia A from the ATHNdataset

Background: The ATHNdataset is a HIPAA-compliant, de-identified database sponsored by the American Thrombosis and Hemostasis Network, including 17,109 hemophilia A patients.
Little is known about range of motion (ROM) in women and girls with hemophilia A

Aims: To explore the ROM in women and girls with hemophilia A using data from the ATHNdataset.

Methods: The ATHNdataset was used to identify female patients between January 1, 2010 and April 30, 2022. Data on 354 patients was available for this analysis (13 female patients).

Baseline demographics, medical history and ROM data were collected.

Results: Five female patients had ROM data available. All 5 patients had mild disease. Baseline FVIII levels ranged from 21 to 41%.
Mean (SD) duration of therapy was 3.6 (2.1) years. Two patients were receiving intermittent prophylaxis, 1 patient continuous prophylaxis and 2 patients were treated on demand.
Three patients were in their 20’s, one in her 40’s, and one was 13 years old when the ROM data was collected.
All five female patients had decreased ROM values when compared to CDC values for normal women.
The reduction in joint ROM was more pronounced in the lower extremity joints compared to the upper extremity joints (data not shown).
This was also observed in the patient who was 13 years old, with some decline in the ROM in her joints.

Conclusion(s): These five women and girls with hemophilia A have reduced ROM compared with normal ROM values used by CDC, despite having near normal FVIII levels
Data obtained from the ATHNdataset demonstrated joint ROM limitations can start at an early age.
The findings suggest female patients with hemophilia may have less joint damage in the upper extremities compared to lower extremities (data not shown).
More data are needed on the joint ROM in women with hemophilia A.
