    Background: Determining the risk of venous thromboembolism (VTE) in first-degree relatives (FDR) of patients with VTE is important for counselling FDR in clinical practice.

    Aims: To evaluate the risk of VTE in FDR of patients with a first VTE (index cases [IC]) compared to that in the general population.

    Methods: Using indirect method, standardized incidence ratio (SIR) and 95% confidence interval (95%CI) were calculated to estimate VTE risk in FDR of IC (Couturaud F. BLOOD 2014) as compared to that in general population from Brest district (Delluc A.Thromb Haemost 2016). VTE events were adjudicated by an independent committee. Participants provided written informed consent. Local ethic boards approved the study.

    Results: Among the 2617 FDR of 507 IC with VTE, 123 had VTE (annual incidence 1.2/1000 person-years); among the 367,911 inhabitants of Brest district, 576 had VTE (annual incidence 1.6/1000 person-years). VTE risk in FDR was higher when IC had unprovoked VTE (SIR 131 [95%CI 109-153]) or had VTE < 47 years (SIR 328 [95%CI 269-385]) or when VTE occurred in ≥2 FDR (SIR 556 [95%CI 484-628]) (Table 1). FDRs with highest risk were when IC had unprovoked VTE and ≥2 FDR had VTE (SIR 636 [95%CI 552-720]) or when IC had VTE < 47 years and ≥2 FDR had VTE (SIR 993 [95%CI 775-1212]). FDRs with the lowest risk were when IC had provoked VTE ≥47 years (SIR 38 [95%CI 9-67]), had provoked VTE and < 2 FDR had VTE (SIR 33 [95%CI 4-63]), or IC had VTE ≥47 years and < 2 FDR had VTE (SIR 49 [95%CI 30-68]). IC’s thrombophilia status did not influence these estimates (Table 2).

    Conclusion(s): The risk of VTE in FDR of IC is predominantly related to the clinical presentation of IC’s VTE. This has the potential to improve prevention VTE strategies, particularly in young women.
