Background: Lung cancer (LC) is one of the leading causes (20%) of cancer death in US and the second most prevalent after breast cancer in females and prostate cancer in males. Patients with non-small cell lung cancer (NSCLC) are at a high risk to develop venous thromboembolism. Profiling of thrombo-inflammatory and coagulation factors may provide better understanding of VTE in these patients.

Aims: This study was designed to profile thrombo-inflammatory and coagulation factors in a clinically confirmed lung cancer population.

Methods: Blood samples from 49 patients of >18 years, and >50 kg with lung cancer were collected under an IRB approved protocol from Loyola University Medical Center. Normal human plasma (NHP, n=50) was used for comparison. Frozen plasma samples were retrospectively analyzed for markers, including D-Dimer, PAI-1, tPA, TAFIa, vWF, CRP, FVIIa, FIX, FX, and FXIIIa using commercially available Sandwich ELISA kits. Results were compiled and applicable statistical analysis was performed by using GraphPad Prism software.

Results: Lung cancer patients showed upregulation of most of the biomarkers, such as D-Dimer (405.6 vs 65.9 ng/ml; p< 0.0001), PAI-1 (19.8 vs 12.9 ng/ml; p=0.0003), tPA (7.8 vs 5.3 ng/ml; p=0.0196), TAFIa (95.1 vs 76.8 %; p< 0.0001), vWF (111.3 vs 98.2 %; p=0.0019), and CRP (4.9 vs 1.8 ug/ml; p=0.0002) whereas FVII (80.5 vs 107.7 %; p< 0.0001), and FXIIIa (77.1 vs 88.4 %; p=0.0125) levels were downregulated compared to NHP. However, no difference was noted in FIX (109.8 vs 110.2 %; p=0.9917), and FX (89.3 vs 89.9 %; p=0.3481) levels. Varying levels of positive and negative correlations between the markers were noted (figure 1).

Conclusion(s): These studies suggest that NSCLC patients are at high risk of thromboembolic complications due to hemostatic dysregulation and fibrinolytic deficit. Profiling of these biomarkers may be helpful in the risk stratification and targeted therapeutic management of NSCLC.
