Background: Impaired health-related quality of life (HrQoL) has been reported in primary ITP patients and used as outcome parameter in clinical ITP studies.

Aims: To provide real-life data on HrQoL in a clinically well-characterized cohort of adult primary ITP patients.

Methods: HrQoL was assessed in 56 adult primary ITP patients (11 (19.6%) acute, 45 (80.4%) persistent/chronic) from the Vienna ITP Biobank (EC 1843/2016) by the short-form 12 (SF-12) and SF-36 Health Surveys and compared to established norm groups (NG).

Results: Patient characteristics and HrQoL data are shown in Table 1. In the SF-12 survey, ITP patients showed physical health scores similar to NG, while their mental health score was significantly lower (Table 1.3 and Fig.1A). Among SF-36 subscales, “General Perception of Health”, “Energy and Vitality”, “Social Functioning”, “Emotional Problems”, and “Mental Health” were significantly impaired in ITP (Table 1.2, Fig.1B). The mental health score (SF-12) was positively correlated with platelet count (r=0.314, p=0.021). Furthermore, both SF-12 mental and physical health scores were negatively correlated with the ITP-SMOG bleeding score (r=-0.320; p=0.018 and r=-0.306; p=0.024).

Mental health (SF-12) was impaired in acute and chronic ITP patients compared to NG (Table 1.3, Fig.1C). In acute ITP patients, our data suggested an impairment, particularly in the “Social Functioning” and “Mental Health” SF-36 subscales (Table 1.2, Fig 1D). Both groups reported impairments in “General Perception of Health”, with statistically significant differences between chronic ITP patients and NG in “Energy and Vitality”, and “Emotional Problems” (Table 1.2, Fig.1D).

Conclusion(s): Overall, HrQoL is impaired in ITP, mainly caused through the patients’ poor mental health. While in acute ITP patients both, physical and mental health was affected, chronic patients suffered especially from low energy and emotional problems, underlining the impact of chronic ITP on patients’ lives.
